Note: Due to signature requirements, University Animal Care cannot accept these forms as e-mail attachments. Animal Order form can be faxed to 520-626-4079. All other forms can also be faxed to 520-626-4079.

It is highly recommended that when accessing non-html file links, such as .doc, PDF formats, etc. that you 'right click' on the appropriate hyperlinks and select 'save target as..' to save these files directly to your hard drive. Note: Depending on the software version you will be using, there may be formatting changes after saving and opening the file. If you need help downloading the forms contact (520)626-2026. It is recommended that you use Internet Explorer version 8X or higher, it works better.
